I don't believe that you can create custom dictionaries for Dictionary.app and when you add new words using spellcheck, they aren't added to Dictionary.app either. The dictionaries used by spell check are separate.
As I understand it Pages uses the same Cocoa spelling check used by many other OS X applications including Mail, TextEdit and Safari. There is one slight difference. In those other applications you are able to select the dictionary to use (i.e. which language) when you choose Edit>Spelling. Pages on the other hand doesn't give you that option and uses the appropriate dictionary for each paragraph based upon the language set in the Text Inspector.
